  • Facilitates referrals and contacts.

Elder information resource

  • Information on a wide range of community resources.
  • Assisting clients with personal goal achievement.
  • Identifying or creating naturally occurring care networks.

Check-in service

  • Scheduled home visits to monitor client well-being, reduce social isolation and promote interdependence.

Advocacy

  • Teach skills in becoming change agents on behalf of self and others.

Written reports

  • Upon request, Consultant will provide written or electronic reports in a timely fashion.
